Advanced Social Worker - Complex Care

The Complex Care Team are part of the Adult Social Care Team within Halton Borough Council. We are responsible for the delivery of social work and care management for adults over the age of 18 (learning disabilities, physical disabilities, and older people). This position in within the Complex Care team in Widnes.

As the Advanced Social Worker in the team you will carry a small case load of complex cases. As an experienced qualified social worker you will be responsible for providing guidance and professional wisdom to colleagues and other professionals. Demonstrating expert and effective practice in complex situations; assessing and managing higher levels of risk and offering expert opinion within the organisation and to others. You will provide regular supervision to team member’s, support the management of Vulnerable Adult investigation’s, chair best interests meetings and case conferences and working closely with colleagues from across health and social care. You will contribute to the continuous improvement of the service, and the on-going integration between health and social care.
