We currently have a 18.5 hour vacancy with the Mental Health Social Care Reviewing Team based at the Coast Resource Centre. Working mostly with adults of working age, you will provide a strength-based social work perspective within a multi-disciplinary setting, based within North Somerset. You will be expected to undertake various duties, primarily Care Act (2014) assessments and reviews working in Partnership with AWP, NHS and service providers. You will ensure service users receive an excellent standard of health and social care. You will work within key legislative frameworks such as the Care Act (2014) and the Care Programme Approach (CPA).
We are now looking for a confident Adult Care Worker, with good time management and communication skills, who can work independently as well as part of a team to make sound evidence based decisions to meet individual’s needs, efficiently and effectively.
