Job Title:Service Finder
Location:Countywide (Predominantly based in Stevenage)
Salary Range: £24,799 progressing to £33,799 pa
Hours: 37 per week
Contract: 1 Year Fixed Term/ Secondment opportunity
We currently have an opportunity for a Service Finder in the Community Solutions Team. The post will be predominantly based at Stevenage, however you may be required to work in Apsley or other areas in Hertfordshire occasionally.
The Service Finding Team support care managers to commission services to meet individual’s needs. Individuals may have a learning disability, physical disability, autism or a combination of needs.
You will be responsible for sourcing; Accommodation with support (residential homes and supported living), Short Breaks, Day Services, Homecare, Community support, Direct payments & Transport
You will need to be able to maintain accurate records, follow business processes and contribute towards process development. Excellent communication, IT, Project management and organisational skills are essential as the ability to work under pressure.
It would be advantageous to have experience of working with people who have learning disability autism and/ or physical disability and a good understanding of what their support may look like.
You will need to develop good working relationships with our social care teams and care providers and be able to build a thorough knowledge of the services they offer.
