Job Details

We are seeking 2 Outreach Support workers to join our Outreach team at Enfield.

One post is on a permanent basis.

Our 2nd post is for secondment opportunity or fixed term basis for 6 months.

Both roles are full time (36 hours a week) and based in Enfield Town.

  • Do you hold a Social Care qualification i.e. NVQ/QCF 3 in Social Care or equivalent?

You must be able to work independently and as part of a team in a busy working environment.

The Outreach Team office is currently based by Enfield Town Train station. The service provides flexible short-term, longer term intervention and supports individuals develop skills and confidence to maximise independence. The service enables those we support to maximise their health and wellbeing.

The post-holders must be able to work independently and as part of a team in a busy environment.

An ability to carry out assessments of an individuals care and support needs and transfer this information in to care plans along with identifying and minimising risks.

You will need excellent communication skills, both verbal and written, to work within the team, and with external customers, professionals service users and their families.
