Assistant Care Manager – Carers Team

Do you want to work within an Adult Social Care environment where teams of professionals are breaking new ground? Do you enjoy working in a solution focused and creative way? This role is to support carers living in Rutland through direct contact with individuals and contribute to the development of services across the county.    


We are looking for a dynamic and motivated individual who can……..

  • Support the independence, physical health and emotional wellbeing of carers and

their families

  • Complete holistic assessments to identify individual needs and circumstances  
  • Provide personlised information, support and signposting
  • Work independently and as part of a multidisciplinary team with carers; relevant agencies

and other professionals

  • Develop and implement approaches to identify ‘hidden carers’ in the community through

innovative engagement
