Assessor

We are looking for an Assessor to join our WISH (Wokingham Integrated Social Care and Health) service in the Health Liaison Team. The post holder will work within an established multi professional statutory team and will undertake assessments for people in acute or community hospitals and requiring discharge on a “home first” basis, working alongside our colleagues in health.. This will be to establish eligibility for Social Care services in line with the Care Act (2014) framework, and will also involve meeting immediate need to manage risk and to ensure a timely discharge from hospital to the community. The role involves working with adults with a broad range of needs including people with learning disabilities, physical disabilities, older people and people with dementia. It also involves completing Carers Assessments and providing information, advice and support to Carers. Our base is at The Forge offices in Wokingham, but during this time home working and use of IT systems such as Teams and Outlook are part of our daily working practices.

 

We are a very supportive team, and offer a structured induction programme, regular supervision, and good opportunities for training.

 

You will need to be flexible and be able to prioritise and manage your time effectively. A car driver is essential as we cover a rural as well as urban area and there is very limited public transport.
