The Community Older Adult Mental Health Service to looking for a qualified social worker, who has experience of working within the mental health field. We welcome applications for both full time and part time applicants and also newly qualified to senior practitioners.

 

The Community Older Adult Mental Health Service is a joint funded service between the local authority and Berkshire Healthcare Foundation Trust. The service has a memory clinic, psychology service, a community team and a Home Treatment Team. Mental Health Practitioners have either a nursing, OT or social work background. We are currently looking for a qualified social worker to work within our community team, who has experience and or a sound knowledge in mental health services and a keen interest in working with older adults who may have either an organic or functional mental health conditions. The post holder will be responsible for managing their own case load, monitoring the mental health needs of patients and working alongside their carers and assessing people under the Care Act. Knowledge of appropriate legislation such as the Care Act, Mental Capacity Act and Mental Health Act is key to this post.


The post holder will be expected to operate within the overall framework of the team’s policies and procedures, including working across both the health and social care client/patient computer systems. Support will be provided through individual and group supervision.


There will be an opportunity/expectation for the successful candidate to train as an Approved Mental Health Professional and Best Interest Assessor.
