Salary: Social Worker £29,577 to £32,234 - Senior Social Worker £32,910 to £35,745

We are looking for enthusiastic and dedicated qualified social workers and senior social workers to join our Adult Community, Hospital, Older People Mental Health and Responsive Neurological Services teams across the county.

Adult Community Teams - Locations: Horsham, Worthing, Bognor Regis

The Adult Community Teams work with people who have experienced cognitive and/or physical disabilities over the age of 25 and primarily with older people. The strengths-based approach is embedded within the service to work with individuals to explore their networks of support in the local community and maximise independence.

The teams have excellent working relationships with Health through the proactive care initiative, where social work representatives regularly factor into local GP surgery multi-disciplinary meetings. Social Workers have a varied workload which includes assessments, reviews, safeguarding enquiries and mental capacity act assessments.

Older People Mental Health Teams - Locations: Horsham, Shoreham, Worthing, Bognor Regis

The Older People's Mental Health Team is a specialist service working with people over the age of 65 who have complex mental health needs. A number of the Social Workers are Approved Mental Health Practitioners and the team work closely with the Dementia Crisis Services and specialist psychiatric wards.

Social Workers utilise their skills to work with customers on a longer-term basis, building relationships where there may be difficulties with engagement, or complexities around the impact of mental disorder. Challenging discrimination and empowering older people to have more autonomy in their lives.

Hospital Teams - Locations: East Surrey, Crawley, PRH Brighton and Worthing

Our busy hospital social work teams support adults across a range of ages with physical disabilities, acquired brain injuries, chronic health conditions, older people and people with dementia. The social workers have fostered excellent working relationships with the multi-disciplinary teams in the hospitals to ensure that people have the most appropriate support when discharged.

The Covid 19 Pandemic has been challenging for our NHS services and the hospital social work teams have been part of a system wide response with more integrated working and the implementation of a discharge to assess model.

Responsive Neurological Services Team - Location: Horsham

Our highly regarded Responsive Neurological Services Multi-disciplinary team (MDT), covers the North of West Sussex thus requiring significant travel around the area. This includes Horsham, Crawley, East Grinstead, Hayward Heath, Burgess Hill and the surrounding areas.

This unique role focusses on strengths-based working, rehabilitation of acute conditions such as stroke and acquired brain injury, and management of progressive conditions such as Parkinson's, MS and Motor Neurone disease with relevant experience of MDT working and / or working with Adults with neurological conditions. You will be working with a specialist MDT including Team Lead, Clinical Lead, SALT, Psychology, Occupational Therapy, Physio and Support Workers.

This means that there is the opportunity to develop within both specialist and non-specialist routes.
