Adult Social Care Assistant

We currently have two, full-time Social Care Assistant vacancies; one at each of our two ASC Guildford locations:

  • Guildford Locality team based at Guildford Borough Council, Millmead, Guildford, Surrey, GU2 4BB
  • Royal Surrey County Hospital, Egerton Road, Guildford, Surrey, GU2 7XX

Guildford Locality Team Vacancy

You will have frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. Your role will be to assist qualified practitioners with a variety of tasks, including assessing people’s needs and carrying out regular reviews.

You will frequently be the first point of contact between the public and the service, gathering initial information and identifying the need for further assessment, as well as signposting people to other services.

You will work in partnership with individuals, their carers, professional staff in other agencies, service providers and Support Brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.

Social Care Assistants will hold a caseload of individuals and carers who will generally have a support plan in place that requires routine monitoring. Applicants will enjoy working with people and have experience of direct work with the public as part of paid or voluntary work, although they may have limited or no experience of visiting people in their own homes or care settings.

You will need to have competent basic IT skills and be able and willing to learn to use a variety of software systems.

The work involves travelling within the county of Surrey. Therefore you will need to be a car driver and have access to a car. This is one of the minimum requirements for this role.

You may work with any combination, or all, of the following care groups: older people, people with physical, sensory, cognitive impairments, people with a learning disability and people with mental health problems.

Royal Surrey County Hospital (RSCH) Vacancy

In this role, you will be based at RSCH, but will work flexibly between all hospitals in South West Surrey.

Following changes within the South West area of Surrey, RSCH have taken over management of the social care function within the three community hospitals in the area: Milford, Haslemere and Farnham.

As such, we have an exciting opportunity where we are looking for staff to work flexibly between the RSCH in Guildford and the three community hospitals. You will receive excellent support and supervision combined with comprehensive training/induction.

Your role will involve a variety of tasks including assessing patient’s needs and working closely with the multi-disciplinary team.

Our aim is to facilitate patient flow by supporting discharges to the destination of choice in a timely manner for social care and privately funded patients by assessments on the ward and liaising with care providers.

You will also be involved in supporting safeguarding investigations and attending safeguarding meetings with the support of qualified practitioners.

As a Social Care Assistant, you will have some experience of working directly with people with social care needs. You will be confident and competent in working with people with a variety of physical and cognitive impairments.

You will need to have competent basic IT skills and be able and willing to learn to use a variety of software systems.

The work involves travelling to other hospitals within South West Surrey therefore you will need to be able to be able to travel to other bases if required. Therefore one of the minimum requirements is that you can drive and have access to a vehicle.

You also need to communicate in a clear, fluent, concise and jargon-free way and in a courteous, calm and professional manner.

You will be working in a team of motivated, dedicated and friendly colleagues and have the bonus of working closely alongside a wider multi-disciplinary team in a thriving hospital environment.
