An opportunity has arisen for a Brokerage Advisor to work with the Personalised Commissioning Team in North Somerset.  The Team works with both adults and parents of disabled children. You will source and match care provisions for Adults of North Somerset regardless of who funds their care. These people may have extremely complex/challenging needs and therefore you will need to be aware of the services in North Somerset that can support people in their own homes and in a Care Home setting.

The ideal candidate would be passionate about supporting people to remain as independent as possible using various services to support them. You will need to be organised, enthusiastic with the ability to problem solve, prioritise and manage workload using your own initiative.
