Herts Valley’s Area (Older Peoples Team) is looking to recruit highly motivated and enthusiastic registered Social Workers. This is an exciting opportunity to join high performing and supportive teams working to support citizens over 65 years who live in a vibrant, multi-cultural area with a wide range of voluntary resources. We are committed to working in collaboration with our key partner agencies (health, social care and mental health services). The aim of this role to promote independence by assessing older people to identify their desired outcomes to maximise independent living whenever possible.
You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being. You will keep the service user at the centre of all activity and address assessed needs within the legislative framework. A good understanding of the relevant adult social care legislation is essential to this post. You will need to be able to work autonomously within agreed timeframes and manage frequently changing situations. You will work as part of an integrated team, attend multi-disciplinary meetings and need to have excellent communication skills. The successful applicant will need to demonstrate sound knowledge and experience of safeguarding practice and skills in risk assessment and positive risk taking which draws on a service user’s strengths and assets. For this role you need to have a recognised qualification in Social Work and be registered with SW England.
