Prevention and Communities Manager

We are looking for tenacious and creative team members to support our Communities and Prevention work within Adult Social Care. In this role, you will build excellent working relationships with partners and residents and will work proactively to co-design and deliver initiatives that support Surrey’s 2030 Vision within the context of health and social care, showing innovation and perseverance to bring about change.

The overall ambition of the role is to support community participation in the delivery of health and social care. The role is based in Adult Social Care, reporting to the Communities and Prevention Lead and will work collaboratively with partners, especially Public Health and the voluntary sector, to develop a range of community resources that enable residents to improve their wellbeing, resilience and independence.

The Directorate provides a range of services to vulnerable adults and older people across Surrey, including information and advice, needs assessments, care management and the provision of day, residential and community-based services.

Social care is facing significant increases in the volume and acuity of people’s need at a time of reductions in local government funding. The care market is becoming increasingly fragile and there have been important changes in national policy. The Care Act 2014 promotes a strengths-based approach to assessment and support planning, requiring local authorities to 'identify an individual's strengths - personal, community and social networks - and maximise those strengths to enable the individual to achieve their desired outcomes’.

To achieve this, the Communities and Prevention Manager will work in collaboration with a broad range of partners, none more important than residents themselves, to embed a culture of collaboration and co-production of social care support. They will create an environment whereby individuals can be diverted towards informal community-based services that prevent, reduce or delay care and support needs from developing, as required by the Care Act 2014. Our ambition is to embed a model of social care that grows local resources and encourages behaviours which benefit the individual, improve the outcomes they achieve and enables the Council to deliver services within the resources available.

We are looking for a dynamic and inspirational person, who can work in a flexible way with communities, staff and partners. The ideal candidate will have experience of innovation and co-design, as well as strong communication, problem-solving and partnership skills.

This role will:

  • Interpret the changing political and social context and identify opportunities for innovation, including liaising with strategy and policy colleagues, and use insight and intelligence to inform decisions and review activity
  • Scope and deliver projects that will grow community skills and strengths-based opportunities for residents, specifically modelling and evaluating an approach to community participation and collaboration that will allow the health and care system to work in partnership with residents as co-producers of health and wellbeing across Surrey.
  • Establish new models of delivery which will allow adult social care in Surrey to deliver better outcomes for residents at less cost, including scoping new approaches to supporting and shaping support opportunities provided by the voluntary, community and faith sector
  • Work alongside social care staff to embed cultural change and best practice, including embedding strengths-based approaches and behaviour change techniques.
  • Work alongside residents to support community capacity building initiatives, such as mobilising a peer-led model of health champions in communities and amongst groups that experience health inequality

The successful candidate will be required to travel around the County to meet the community and partners in various locations. In line with SCC policy, efforts will be made to reduce the travel required where possible.

The role is based at one of Surrey County Council's offices across Surrey. This is flexible and will be discussed at interview.
