Job Title: Project Manager
Salary Range: £35,745 progressing to £46,845 per annum
Hours: 37 per week
Contract: Fixed Term
The Integrated Care Programme Team is looking for forward-thinking candidates who have proven project management experience, desirably delivery major changes projects in social care or the NHS, and a passion for delivering change for our residents including frail older people and adults with learning disabilities or physical disabilities. We are seeking someone who confidently develops relationships, can collaborate across with a wide range of stakeholders; and who can deliver multiple and complex projects translating strategy and ideas into action and change.
Hertfordshire is at the forefront of delivering new models of health and social care and a number of our services have won national awards. We have agreed an ambitious vision for integrated care, set out in our Better Care Fund strategy and aligned to the Hertfordshire and West Integrated Care System (ICS).
This position will play a role in leading transformation across social care and health including reducing health inequalities for people with a learning disability and a range of other integration opportunities. Usually this role would at times require you to work anywhere across Hertfordshire, working with a range of partners including the Clinical Commissioning Groups, health and care providers, and our communities including the voluntary sector. However, at present the team are fully remote working at home until further notice.
