An exciting opportunity to work as part of the NHS in an  Integrated  Community Health and Social Care service. Adult social care has a number of Qualified Experienced Social Worker vacancies across the Wirral as part of the Integrated care coordination teams. Working as part of a multi disciplinary team in a community setting. Proven experience of working with adults over the age of 18 years. Experience and knowledge of working with individuals with multiple conditions, Learning disabilities, elderly frail and  younger people .  Must have BIA qualification and  Practice Educator award . Ability to support colleagues and promote professional standards is integral to this post


Proven knowledge of the Care Act 2014, Mental Capacity act 2005, Safeguarding and other specific legislation appropriate  Adult Social care is essential .
