Job Title: Social Worker – Herts Valleys Integrated Hospital Discharge Service
Location: Various sites covering Herts Valleys, including Watford, Apsley and Barnet
Salary Range: H8 – M1 £27,741 - £39,880pa (dependent on experience) plus relocation package, £1000 welcome payment & lease car* terms apply plus £4,000 Market Forces payment for Social Workers based at Watford General Hospital & Barnet Hospital Teams and £2,000 Market Forces Payment for Social Workers based at all other hospital teams.
Hours: 37 hours per week

What does the role involve?
The service supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.
We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.
What is required?
You will need to be SWE registered with a Social Work Degree. You must have knowledge and experience of relevant legislation.
You should be able to demonstrate a commitment to supporting people with the right care and information at the right time to help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to within our Connected Lives Framework (Your connected life | Hertfordshire County Council).
To improve outcomes for service users, the Integrated Discharge Service supports a 7-day service. You will be asked to part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after. There will be opportunities for you to discuss this at interview if your application is successful.
A driving licence would be advantageous as is the ability to respond to situations quickly and flexibly.

In light of this, the ability to drive and/or travel around the County is required for this role a DVLA check will be required on appointment.
