This is an opportunity for a community care worker to work within the Occupational Therapy service, part of the Initial Assessment Team. It is a multidisciplinary team providing occupational therapy and social work assessment to Halton residents. The team has responsibility for providing equipment, adapting homes and providing care management for vulnerable and disabled people in Halton. The occupational therapy part of the team has a range of responsibilities including:
- Screening of referrals
- Assessment of the long-term needs of residents who have a permanent and substantial disability for equipment, minor and major adaptations to their home that will increase their level of independence
- Carrying out blue badge assessments
We are seeking to recruit an Occupational Therapy Community Care Worker who is interested in working specifically with clients and their families, providing a high quality service aimed at improving their independence at home. You will be visiting clients in their own homes, assessing their needs, providing information, advice, equipment and, where necessary, recommending adaptations – all with the aim of helping them to be independent at home and supporting their families.
It is a busy service with opportunities to gain a wide range of experience in all aspects of equipment and adaptation provision. You must have direct experience with individuals with physical and sensory disabilities. The team has excellent working relationships with colleagues in the council, and in health and housing services.

You should have direct experience with individuals with physical and sensory disabilities, and experience of care management. You should have a full driving license.
