Job Title: Commissioning Manager
Salary range: £43,857 pa - £46,845 pa
Hours: 37 hours
Contract: 2 year Fixed Term
The Integrated Accommodation Commissioning Team is recruiting a Commissioning Manager to review and develop pathway 2 Discharge to Assess services commissioned by Hertfordshire County Council in partnership with the NHS.
This role is central to ensuring positive outcomes for frail adults leaving hospital, to support care system recovery during the COVID 19 pandemic, and to Hertfordshire’s long-term commitment to delivering a true Discharge to Assess pathway.
The post holder will combine an ability to bring significant improvements to commissioned pathway 2 Discharge to Assess services with a passion for achieving the best outcomes for people with care needs. The ability to work closely with all partners within the integrated care system will be critical.
