1 x Permanent & 1 x 15-month Fixed Term

We are looking for two Programme Managers to join our team in Health and Social Care Integration.

Could you be the person to help us improve our citizens’ quality of life and wellbeing by leading the integration of health and social care across Hertfordshire?

Hertfordshire is at the forefront of delivering new models of health and social care nationally. We have an ambitious vision for integrated care, placing our residents and communities at the centre of service delivery and making sure that their care and support is accessible, joined-up and personalised. 

To build on our strong track record of delivering integrated services to our citizens, we are seeking two individuals with excellent programme management and strong partnership working experience to work across the NHS and the County Council to deliver a programme for integrated health and social care.

You will manage a team of projects managers and deliver on a range of projects and priorities to support the NHS and the County Council to help people to live more independently for longer, including:

  • The delivery of Hertfordshire’s Better Care Fund Plan and its strategic ambitions for health and social care integration
  • The development of integrated health and social care community teams
  • Improving flow and reducing delayed transfers of care
  • Reducing demand on urgent care services through prevention
  • Joining up IT and Data to inform decisions and commissioning

We are looking for an individual with highly developed programme management skills and the ability to influence, motivate and inspire colleagues across health and social care.
