As such, we have an exciting opportunity where we are looking for staff to work flexibly between all of the hospitals. You will receive excellent support and supervision combined with comprehensive training/induction.

You will work closely with the multi-disciplinary team to establish discharge plans and follow up on patients following discharge to reassess their ongoing needs.

You will need to have competent basic IT skills and be able and willing to learn to use a variety of software systems.

The work involves travelling within South West Surrey and also to other parts of the county. We therefore ask that you are a car driver and have access to a car.

You need to be able to communicate in a clear, fluent, concise and jargon free way and in a courteous, calm and professional manner.

You will be working in a team of motivated, dedicated and friendly colleagues and have an added bonus of working closely alongside a wider multi-disciplinary team in a thriving hospital environment.

The role involves working in a rural location when at two of the hospitals. All three community hospitals offer free parking and there is paid for parking within Royal Surrey County Hospital.
