Early Help and Prevention/SEND
The role will be to ensure that the care needs of children with special educational needs and disabilities are clearly collated and documented within the child’s Education, Health and Care Plan.
The post-holder will act as the single point of contact when care advice is required by the Local Authority and ensure the most appropriate practitioner(s) provide the advice whether this is the child’s Social Worker or Lead Practitioner.
The post holder will need a minimum NVQ level 3 or equivalent in a related field (e.g. Health and Social Care) or equivalent experience
They must have a minimum of 2 years, recent experience of working directly with families with multiple needs with children with disabilities.
