Adults Assistant Team Manager

The starting salary for this role is £45,734 per annum, working 36 hours per week.

The Transition Team, with office bases in Woking and Redhill, is a dynamic and vibrant countywide service for young people aged between 14 and 25. Although based within Adult Social Care, our journey with the young person and their family starts much earlier at academic year 9.

We work with young people who have a wide range of additional needs and complex conditions. We challenge inequality and traditional responses to service delivery and ensure that we employ a strength-based approach to our work. Supporting them and their family through the Preparation for Adulthood agenda, we ensure that young people have control of their life outcomes and aspirational goals that will support them into employment pathways, independent living, and meaningful occupation. Our aim is to provide a smooth, well planned transition into young adulthood for the families we support.

We are remodelling approaches from traditional institutional approaches as we challenge inequalities that prevent young people with disabilities and their carers from living a life free of discrimination and limitations.

We understand the challenges of day-to-day occupational therapy and frontline social work and have put tools in place to improve your work/life balance and make day-to-day tasks more manageable. We provide hybrid devices with a touch screen, detachable keyboard which enable staff to access council systems when working directly with individuals. These devices allow colleagues to work in a more agile way, working from home, office, or other locations as appropriate.  

The Occupational Therapy service in the Team

We have a small, stable, and experienced Occupational Therapy Service within the Transition Team and these exciting opportunities do not come up often. We work closely with our OT colleagues in Children’s services, however our intervention and support are currently focussed on supporting young adults aged 18-25. This provides an exciting opportunity to support young adults at a significant developmental period of their lives.

We visit young people in their own homes, in tenancy based supported living, care homes and in educational establishments, including colleges and universities. We carry out assessments and give advice regarding complex moving and handling arrangements, provision of specialist equipment and property adaptation and design. We provide advice and support to both young adults, their family carers and independent support providers – often enabling them to move on to a more independent lifestyle.

About you

We are looking to develop our Occupational Therapy Service to focus on maximising the potential of young adults and your professional creativity on developing this will be key to your success.

As Assistant Team Manager / Advanced Practitioner (OT), you will be a professionally qualified and registered Occupational Therapist with a proactive, person-centred, strengths-based approach to your work, a supportive team member and able to offer advice and guidance to OT and non-OT colleagues. You will have a minimum of four years contemporary post-registration experience, including routine experience of complex casework, risk assessment and safeguarding. Knowledge of social care policy and legislation is essential, as is a background in working directly with people with variable, unstable or challenging social care needs and clinical conditions. Experience of direct work with young people and young adults is desirable. You will have an eye for detail but also the ability to think outside of the box to co-design creative solutions with young people, which may sometimes be viewed as unconventional. The ability to get the best out of others and work collaboratively with partner agencies is equally important, and you must have outstanding assessment skills. You will need to have competent IT skills and be able and willing to learn to use a variety of software systems.

As Assistant Team Manager you will have day-to-day operational and clinical management responsibility for our Occupational Therapy service within a large, busy, dynamic team. You will be supporting the Team Manager to deliver high quality personalised adult social care, with a clear strength-based agenda and bring the Occupational Therapy perspective to strategic team planning and management decisions. You will be an experienced supervisor and provide clinical supervision to the Senior OTs within the team. We also regularly provide support to Trainee and Student OTs.

You will lead on the development of the service developing effective partnership relationships with young people, carers and families and positive relationships with health colleagues.

As an Advanced Practitioner you will be an excellent communicator and have proven skills and experience in major housing adaptations and complex moving and handling assessments. You will maintain and develop your professional casework skills, by being the keyworker for, and supporting your colleagues with, complex cases, modelling best practice, and providing expert advice and support within the team.

Many of the young adults we support have a degree of cognitive impairment and an applied knowledge of the Mental Capacity Act will, therefore, be essential. You will also be able to demonstrate an applied knowledge of the Care Act, the Mental Capacity Act, and the NHS Continuing Healthcare National Framework and have excellent Safeguarding knowledge and skills.
