Salary Social Worker Level 1: £28,099 – £30,719 per annum pro rata + MRS £1,000 per annum pro rata

Contract: Permanent

Hours: Full-time - 37 hours per week

Base Location: Priory House, Shefford with travel to Various places throughout Central Bedfordshire, with some home working

More about the role:

 As a Hospital Social Worker, you’ll be at the heart of our Better Care Fund developments whilst supporting our residents to leave hospital safely. Join us and find your greatness in supporting vulnerable people to be safe, well and as independent as possible.  

Our Hospital Discharge Service provides Assessment and Case Management across all client groups. We are committed in supporting people to leave both acute and non-acute hospital settings with timely and safe integrated discharge planning.  Central Bedfordshire has a number of positions to support people across multiple hospital services that serve our Council area.  
The service works in close partnership with other health and care systems, we work alongside our Community Health Service provider to deliver a multi-disciplinary approach to Integrated Health & Social Care. The working week of 37 hours is spread across 7 days.

You will have significant Health and/or Social Care experience, including multi-disciplinary and inter agency working. Your ability to transfer your skills will help us achieve our goal in the delivery of excellent person-centred approaches. You will know the importance of enabling people to be supported to be as independent as possible for as long as possible and that having a strong connection with their local communities can make a real difference to people's lives and overall wellbeing.
