Social Worker (Frailty) - Qualified and Experienced
Full Time (36 hrs) based at Southport Hospital

An exciting opportunity has arisen in Adult Social Care within Sefton Council.
As a qualified social worker in a busy Frailty role, you will be responsible for fulfilling the Local Authorities statutory duties under the Care Act. This includes assessing individuals and arranging practical and social support for vulnerable people to avoid hospital admission from both a community setting, but also to avoid hospital admission from Southport and Formby general accident and emergency department.
You will have knowledge of ICRAS and be ready to manage a defined case load delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual packages of care, whilst upper holding the excellent reputation of the Council.
