Salary: Starting from £25,419 to £30,095 per annum
Grade: Grade 5
Hours: 37 hours per week
Place of work: Based in GP surgeries and community locations across Adur and Worthing, with remote and home working included

Going Local has been operating in Adur and Worthing since 2016. Thousands of people have been referred to find out ‘what matters to them’, as opposed to ‘what is the matter with them’, enabling them to deal with issues that often include housing, mental health, money advice, lifestyle issues, loneliness, volunteering and addiction support, amongst many others.
We’re looking for a triage Social Prescriber to join the team for a 12 month period.
Being an integrated part of the GP practices and the wider Primary Care Network family, the post-holder will be expected to work flexibly, across a number of sites and be confident in building rapport with staff at all levels.
If you’re passionate about people and have excellent core communication skills for working with our service-users and our many partners, this could be the job for you!
You’ll need to be good at prioritising, efficient with your time, a good navigator - of both people and agencies and tenacious - we need clear case-notes, good diary management and attention to detail.
You must have client-facing experience as well as sound motivational interviewing skills and be a great ‘active’ listener. We’ll help you develop these and other skills, making sure we’re the best for the people we’re supporting in our local communities and that your personal development continues to flourish.
