Working as a Senior Social Worker for Reading Borough Council you will be part of a friendly and supporting team.  We are looking for an individual who can demonstrate great communication skills and the ability to build and maintain relationships with colleagues, young adults and health care providers.

About the Role:

This is a role for a Senior Social Worker who is interested in working with 14 to 25 year olds in the PFA team. 

Team Objectives:

  • To provide a timely transition for all Care Act eligible 14 to18 year olds into adulthood and ensuring all 18 to 25 year olds are receiving the appropriate ASC support under care act eligibility criteria.
  • To attend all 14 to 16 year old EHCP annual reviews once consent gained, to begin/review joint working with children’s services from 16 to 18 year olds, and to take over all eligible support and funding for 18 to 25 year olds.
  • To promote as much independence, whatever that looks like to the young adult, as possible to allow for successful and achievable outcomes for our young adults.
  • To embed a reablement focus for the young adults.
  • To work with all families to ensure they understand the role and remit of PFA.
  • To develop close and effective relationships with providers, community and voluntary services, children’s services, schools and colleges.
  • To be a high performing, efficient, supportive and outcome focused team who together embed the principles of TEAM Reading.

 

You will hold a varied caseload and contribute to the team performance and positive outcomes for our residents.

You will have the opportunity to improve your skills in high quality, client-led, interventions to young people in their own homes and community, in a friendly and supportive team.

You will work within the legal framework provided by the statute, guidance, policies and procedures.

You will need to have:

  • A degree or equivalent in Social Work
  • Registration with Social Work England
  • Understanding of the Care Act 2014 and the Childrens and Families Act 2014

This role offers Agile/Hybrid Working – you have the opportunity to work from home and the Civic Offices in Reading but also you will need to attend face to face visits within the Reading Borough.
