Community Learning Disability Nurse

An exciting opportunity has arisen. The Halton Adult Learning Disability Nursing team are looking to recruit an experienced, registered learning disability nurse for our Community Adult Learning Disability nursing team in Halton.

 

The team works with individuals who have a learning disability, with additional complex needs, which may include challenging behaviour, physical and sensory impairment, and mental health. We work with adults with learning disabilities, families and carers from age 18 upwards across the borough of Halton.

 

The post-holder will be responsible for the assessment of health care needs for individuals referred to the Service. The post-holder will have responsibility for prioritising and managing caseload delegated by the Team Manager and be responsible and accountable both professionally and legally for all aspects of clinical and non-clinical activity in the service area, ensuring a high standard of clinical care is delivered to the people with learning disabilities under their management. The post-holder will be required to work in a range of environments away from the team base. These may include individual’s homes, hospital, GP surgery, school, college or day service and will not be directly supervised but will have support from the team.

 

The team are committed to improving outcomes for individuals with a learning disability across health, social care and third sector agencies.  So if you are a registered learning disability nurse that has the relevant knowledge, skills and experience and is a person centred individual then we would love to hear from you.

 

The role will involve regular travel across the borough and sometimes further afield. Therefore, the post holder must have a driving licence and access to a vehicle. Where necessary, reasonable adjustments will be made in accordance with the Equality Act (2010).
