Full time - 37.5 hours per week (working flexible shift patterns to cover a variety of hours between 8am to 8pm across 7 day service, weekends on a rota basis. For every weekend worked as part of your contracted hours, you will get the time back in the week – Enhanced rates will be paid for any weekends or bank holidays worked.)

Main duties of the job

 

An opportunity has arisen for a dynamic and motivated, experienced, band 6 Social worker to join a busy, pro active, multi- disciplinary team within the Trust.

Full and part time vacancies are available.

You will work with people aged 18+ who may have multiple conditions, learning disabilities and complex care needs as part of a multi disciplinary team in a community setting.

To join our Community Social Work teams you will:

  • Be a registered Social Worker with Social Work England
  • Have completed ASYE and critical reflection modules post qualification
  • Have proven knowledge of the Care Act 2014, Mental Capacity act 2005, Safeguarding and other specific legislation appropriate to  Adult Social care
  • Be committed to delivering a high standard of care and promoting professional standards
  • Be an excellent communicator who will innovate, inspire and support colleagues
  • Advocate for members of our community, identifying changes in need and planning for their current and future care

This post requires applicants to be at a social worker level having completed your ASYE.

As a social worker in the Trust you will be working with a model of strength based social work practice that enables you to make a difference to the lives of the people we support at right time.

Current Social Work England registration with the relevant social work qualification is essential to this post. Travel is an essential part of this role and therefore access to a car for work purposes is essential. Qualifications

Essential criteria
  • Social Work qualification e.g. Degree in Social Work; Dip.
  • Be registered with the Social Work England and able to evidence
  • Evidence of continuing professional development in line with Professional Competencies Framework.

Experience and Knowladge

Essential criteria
  • Completion of Assessed and Supported Year Experience plus 2 years experience
  • Able to demonstrate practice which supports the core social work values and principles.
  • Proven experience of working with vulnerable adults and carers.
  • Knowledge of legislation relevant to Adult Social Care and work
  • Understanding of confidentiality and data protection requirements.
  • Experience of completing safeguarding enquires and making safeguarding personal. Undertaking statutory duties and coordinating
  • Awareness of current practice issues and challenges facing Social Workers.
  • Commitment to anti-discriminatory and anti-racist practice.
  • Commitment for continuing professional development.
  • Experience of working in partnership with individuals, families, carers colleagues and partner agencies both voluntary and publicsector.
  • Experience of arranging support plans and/or packages of care.
  • Experience of working with adults with complex needs.
  • Experience of multi-disciplinary working.
Desirable criteria
  • The ability to prepare statement or reports for court proceedings. This could include attendance in court
  • Requirement to undertake the Best Interest Assessor training
  • Required to undertake the practice educator award.

Skills & Attributes

Essential criteria
  • Ability to communication and engage with people in highly complex/challenging situations using a range of interventions effectively and the ability to reflect and evaluate practices.
  • Able to communicate verbally and in writing with a diverse range of people including families and professionals.
  • Able to demonstrate organisational skills
  • Sound assessment and interviewing skills
  • Demonstrate appropriate IT skills e.g. use of Microsoft Office etc.
  • The ability to use formal and informal supervision to reflect on practice.

Personal Qualities

Essential criteria
  • Able to travel within or outside the Borough. Must be willing to move across teams according to the needs of the service
  • Able to work five out of seven days a week on a rota basis. This includes weekends and unsocial hours
  • Exercise emotional intelligence and demonstrate a developing emotional resilience in relation to their job role.
  • Able to work autonomously and seek out support when required
