Adults Senior Social Care Assistant

The starting salary for these positions is £29,291 per annum for working 36 hours per week.

We are looking to recruit two Senior Social Care Assistants to be based at our Spelthorne Locality Team, Ashford Centre, Stanwell Road, Ashford, Middlesex, TW15 3DU.

We are creating a new team to support our discharge to assess and recover, bed based approach. The team will be based at Spelthorne Locality Team, however, there will be an expectation that the team are embedded working within the services.

This is an exciting opportunity for people who:

  • Have great communication skills
  • Can build relationships quickly to utilise and develop their own skills and expertise
  • Have a particular interest and aptitude to work with people and families within an agreed timeframe in a strength-based way

Working across Runnymede, Spelthorne, Woking and West Elmbridge, the North West Surrey Alliance have developed an innovative approach to support Discharge to Assess and aid recovery from Ashford and St Peter’s Hospital.

People who would benefit from bed based Reablement, rehabilitation or an opportunity to recover whilst waiting for their package of care to start will be offered a short stay in one of three local nursing homes or an independent step-down flat for up to 4 weeks before returning back home. These have been developed in partnership with the private nursing home market and with District and Borough Council colleagues to enable people to recover and regain skills and return home.

We are recruiting to a small team of people to specifically support these people to return home following their short stay, to enable assessments and support planning to take place, building on a person’s own strengths with their families and support networks.

You will be part of a team and will have access to direction, support and supervision from a qualified Social Worker. You will have frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. Your role will involve a variety of tasks including assessing people's needs and carrying out regular reviews. You will work in partnership with individuals, their carers, professional staff in other agencies, service providers, and Support Brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.

You may work with any combination, or all, of the following care groups: older people; people with physical, sensory, cognitive impairments, people with a learning disability and people with mental health problems.

To be shortlisted to interview for this position your application will clearly evidence:

  • Substantial experience of working directly with people with social care needs, including completing strength-based assessments and visiting them in their own homes
  • Confidence and competence in working with people with a variety of physical and cognitive impairments
  • Competence in basic IT skills with a willingness to learn how to use a variety of different software systems

Applicants must hold a valid driving licence to drive in the UK and have access to a car.
