We are looking to recruit to the position of a Homeless Reconnection Officer to support the Royal Free London hospital team in the timely discharge of rough sleepers into short-term step-down accommodation. This is a 12-month secondment opportunity as part of the Department Health & Social Care winter funding initiative.
You will work alongside the hospital discharge team in identifying homeless rough sleepers who are ready for discharge. You will be responsible for developing a discharge plan with the individual, support them into step down accommodation at Olallo House and then work intensively with the individual to address their health and social care needs as well as secure more long-term accommodation
This post is exempt from the Rehabilitation of Offenders Act and will require an Enhanced DBS.
We are looking for someone who has a passion for working with homeless people and ideally with experience and knowledge of Camden’s housing pathways.
You will have a working knowledge of housing and homelessness legislation as well as housing options for homeless adults. You will have an awareness of services available to rough sleepers, homeless applicants and vulnerable adults and experience of making referrals to both statutory and voluntary agencies.
Additionally, you’ll have the ability to assess risk in a variety of settings and scenarios. Ability to apply this skill in documented form and in dynamic and changing circumstances on a day-to-day basis.
