Community Care Officer

Hospital – East and North Hertfordshire

  • Integrated Discharge - Princess Alexandra Hospital

Princess Alexander hospital is based in Harlow Essex we support Hertfordshire residents to return home safely.  Working with their strengths, community resources and if needed a support package. We work with the multi-disciplinary team to understand the needs of our service users that we work with.  If you are interest in working in a hospital environment and are prepared to work quickly and learn a lot this job may suit you.

  • Post hospital review team

You will be allocated cases to review care arranged on hospital Discharge to support and enable the person to regain their skills and independence once in the community. We link and work closely with enablement providers and enablement Occupational Therapists to support the person through their social care journey to support the person to achieve their goals in line with the care act 2014 and enablement goals. We utilise community resources and assistive technology to promote a person’s independence and least restrictive support whilst promoting personalised assessments and care and support planning. It is essential to have an understanding of the care act.

  • Community Hospital / Emergency Discharge Lister / Lister hospital

Lister is a busy, dedicated team, supporting residents of Hertfordshire home from hospital as quickly and safely as possible. Lister sits within the  East and North Herts Integrated Discharge Service supporting people home from acute hospitals, community hospitals and Post discharge review.  To improve outcomes for service users, East and North Herts Integrated discharge team support a 7-day service.  The successful candidate will therefore be asked to work on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering a weekend. The successful applicant will be able to evidence sound knowledge of safeguarding practice, key legislation, working with people with dementia, the personalisation agenda, and Individual Budgets whilst promoting and supporting  people to achieve their desired outcomes.

Adult Disability Service East & North Herts Hertfordshire

  • Review Team

Supporting adults with a learning or physical disability we promote personalisation through our new assessment process focusing on the wellbeing of the individual.  Through care planning we strive for increased independence, choice and control by offering personal budgets to enable people to direct their own support. 

We are looking for a Community Care Officer who will assess and review the needs of people receiving a service.  You will provide information on services and liaise with service providers and other agencies as appropriate. The successful candidate will have the ability to gather, record and provide information electronically, to implement care management.

We are based in Stevenage and you would be required to be office based 40of the week with the option of working at home or at one of our touchdown sites. 

Hospital – West Hertfordshire

  • Watford hospital 

Watford is a busy, progressive team, supporting residents of Hertfordshire home from hospital as quickly and safely as possible. Watford sits within the wider Herts Valleys Hospital Discharge Service supporting people home from acute hospitals, community hospitals and their post discharge follow up. To improve outcomes for service users, Herts Valleys Hospital Discharge Service support a 7-day service.  The successful candidate will therefore be asked to work on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering a weekend. The successful applicant will be able to evidence sound knowledge of safeguarding practice, key legislation, working with people with dementia, the personalisation agenda, and Individual Budgets.
