This team works closely with Hillingdon hospital and out of borough hospitals, alongside the CCG to support with hospital discharges either back to the community with a package of care or to a placement. The team respond to safeguarding enquiries, carry out best interest assessments for service users, Implementing and reviewing care packages in the community (Service users homes / Supported Living / Care Homes) and ensure referrals to supported living, residential or nursing homes. Although the role is based with the hospital team the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers.

You will be a qualified, experienced Social Worker looking to progress your career or an accomplished Senior Social Worker, with a wide range of experience working with Older Adults and/or People with Disabilities. You will have developed your skills and knowledge with further post qualifying training and have specific experience with the relevant client group.
You will be innovative and creative with a willingness to explore new models of working to improve the outcomes for our service users. Moreover, you will be a highly motivated individual who is able to build strong relationships to deliver the best possible outcomes for vulnerable people.
