We have 2 vacancies for Consultant Social Worker roles within the 0-25 Together Service. Both roles offer exciting opportunities to work closely with disabled children in partnership with their families and professionals.  One role sits within our West Locality Team where you will play a key role supporting the team manager in the day to day activity of running a busy and vibrant team. You will have a manageable caseload and supervise a small pool of family practitioners.

The other role will sit in our newly formed Preparing for Adulthood Team. Here, you will work closely with the team manager, supporting Preparing for Adulthood practitioners, guiding practice and ensuring proper application of Child In Need procedures in the context of young people being prepared for adulthood.  You will also work closely with our adult social workers and ensure that a young person’s journey into adulthood is as seamless as possible, keeping the young person at the centre of the process

As our Consultant Social Worker in 0-25, you will lead best practice. Your knowledge, skills and experience will demonstrate your understanding of the legal framework and multi disciplinary nature of our work to support Disabled Children and their Families. Regardless of which team you join, you will assess the needs and circumstances of an interesting range of cases, endeavouring to help people establish independence (either now or in the future) while balancing this with safeguarding considerations. Providing a great level of support to service users’ support networks, you will advise them on resources within the community that may be helpful, and you will collaborate with both statutory and voluntary agencies to deliver great life outcomes. Every day will present you with new challenges, as you could handle anything from making recommendations on home adaptations through to enrolling service users in enabling programmes. From your initial assessments through to overseeing the planning and delivery of services – and then monitoring the success of these measures – you will play a vital role in keeping people happy and safe.

Technical, skills and qualifications-

• Qualified Social Worker registered with Social Work England
• Experience of working with children and young people
• Knowledge of legislation, guidance, policy, and procedures
• Ability to complete robust assessments & to assess risk
• Ability to work in a relationship-based way
• Understanding of equality and diversity in Social Work
• Ability to input and maintain accurate computerised information and records
• Ability to work on own initiative managing and prioritising own workload under pressure and under the direction of the supervisor

Desirable

• Knowledge/experience of working with Disabled Children and Young People
• Knowledge of legislation, guidance, policy, and procedure specific to Disabled Children and Young People
• For the PFA role, knowledge/experience of legislation, guidance, policy and procedure specific to preparing disabled young people for adulthood.
